Moving on to the adult clinic is a team effort and takes time.

A big step

This is a big step. It is normal to have mixed feelings about the change. For example, you can be feeling excited but also scared at the same time.

Other young people like yourself say the best thing about the adult clinic is that the doctors and the nurses focus on what you need as a grown up.

How does it work?

You will also start having your appointments at the adult clinic between the ages of 15-18 years.

Your doctor and nurse at the children’s clinic will start to prepare you for the adult clinic. This period is called transition. During the transition, you will more and more have opportunities to make decisions for yourself and take over the responsibility for your health. 

The goal is to help you become independent and responsible for your own health care.

Your medical team will talk to you alone without your parents or caregivers about your health care and needs. If you want, you can get your own personal copy of clinic letters.

They encourage you to be responsible for remembering to take your own medication.

Moving on to the adult clinic will only happen if you are ready. Talk to your medical team if you are scared and don’t feel ready.

Remember, it is your health-care appointment. Your questions and worries are the focus of your visit.
